Your Medical Home

Your Patient Centered Medical Home (PCMH) or “medical home” for short, is a health care model developed by the American Academy of Pediatrics (AAP) over 20 years ago. The concept continues to be relevant today as the AAP advocates for its place in the “big picture” of health care reform. Its basic premise is that personalized, coordinated, and comprehensive patient care managed by a personal physician will lead to improved health outcomes (quality and safety) and lower health care costs.

The idea is, from start to finish, Genesis Pediatric Medicine is the home for all of your child’s health information. This way, you can be confident that your child’s personal physician has the complete picture of your child’s care and can help you manage and coordinate it. Although it seems this should always be the standard of care, it is difficult to achieve and therefore remains elusive for most pediatric practices.
